This article answers the debate over the use of heat and cold for injuries. It explains the effects on the body of both and when and for what purposes they should be correctly used.

Hot or Cold. The Burning Question.

Hot or Cold. The Burning Question.Copyright 2006 Robert RylesShould You Blow Hot or Cold?As a Physical Therapist I am often asked:"Is it better to put heat or cold on an injury?"A really good question in my opinion. It is, however, not surprising that there is uncertainty regarding this. Instant ice packs and chemical cold packs together with hot packs and microwaveable wheat bags are freely available today. All of these come with the manufacturer's claims of pain relief and physical benefits of their use. Being that essentially, on the surface anyway, the two are diamatrically opposite, how can they both be beneficial? If one is good for you is it not likely then that the other is not? Seems sensible.Ok. Let's look at both of these modalities of injury treatment and see if we can throw some light on to this debate.HEATWhen heat is applied to the body the following occurs. There is an increase in what is known as local physiology at the site of application. In essence this means that there is a greater amount of bodily activity occurring. Blood flow to the heated area is increased. There is a greater demand for and use of oxygen and nutrients. There are more waste products created. There is a calming or sedative effect on nerve endings in the area.So when and how then is heat useful to us as a tool in the treatment of injury? It is useful to help reduce pain as it has a sedative effect on nerve endings. At the same time it has the effect of increasing local tissue metabolism or physiology. This means it is useful to help stimulate blood flow and bodily activity to promote the healing process by making available the maximum amount of blood, bringing oxygen and nutrients. It also has the effect of being a counter irritant. This means it has the effect of being an effective form of pain relief because the sensation of heat overrides the sensation of pain in the body's nervous system.COLDWhen ice or cold is applied to the body the following occurs:There is a decrease in the physiology or metabolism in the area of application. The blood vessels tend to shut down to lessen the blood flow to prevent heat loss. The demand for oxygen and nutrients is lessened and there is a decrease in waste production. There is also an associated pain relieving phenomenon. So when and how is cold useful? As it is able to reduce pain, local metabolic activity including blood flow, tissue demands and waste production. it is useful when the desire is to shut down the body's processes. From this analysis of the effects of heat and ice it is obvious that both have a pain relieving capacity. Part from this though the two modalities create totally opposite effects. Heat increases local tissue metabolism and stokes up the fires of activity whilst cold or ice tends to shut everything down.At the point of injury or in the following few days when the condition is still new the aim is to prevent further tissue damage, reduce swelling and shut down as far as possible the activity of the body to allow the injured area to rest before it begins, in earnest, its recovery process. This is when ice or cold therapy is so important. It is only relatively recently that it has been realised how important this is in preventing further problems and even in severe traumas, death. It was discovered during military conflicts that wounded soldiers found, rescued and returned to base wrapped up in blankets and kept warm tended to die where as those initially missed or left out injured in the field to face the cold tended to survive. It was the effect of heat and cold that influenced the outcomes in many cases. The injured soldiers were only able to survive in many cases because the metabolic demands of the body were markedly decreased by the effect of the cold. So cold or ice is fantastic for new injuries to help relieve pain and prevent further swelling and damage.Heat applied at the time of injury will only tend to make matters worse by increasing blood flow, swelling and tissue demands. The story changes somewhat as the injury settles and the healing process begins. After approximately seventy two hours the injured area will normally have settled down and bleeding will have stopped. Heat may be used after this point to relieve pain and stimulate blood flow to enhance the healing process. It is important that it is not applied too soon and relevant expert advice should always be sought regarding this but the seventy two hour rule is a good guide.Heat and ice are both highly effective but can also be dangerous, causing burns and other problems. It is advisable to learn when and how to correctly use these modalities. There is a free and importanat tutorial on this on the website. So hopefully it is clear.. The definitive answer to hot and cold.

Managing Side Effects of Birth Control Pills

Managing Side Effects of Birth Control Pills

Birth control pills or hormonal oral contraception, as they are also known, are cited for different side effects like lighter and regular periods, clearer skin, less menstrual cramping, and decreased risks for certain cancers. However, different women may have varying reactions to the hormones in oral contraceptives. As such, the side effects of birth control pills can range from positive to negative depending on the body's reactions to the artificial hormones. Below are some of the not-so-nice side effects of oral contraception and ways to manage them: Light, in-between period bleedingSome women may experience bleeding in-between periods when they first start using oral contraceptives. While uncomfortable, this side effect is only a sign that the body is adjusting to the introduction of additional hormones. This side effect may last for up to three months but will eventually pass. Using low-absorbency tampons, sanitary pads, and panty liners can make in-between period bleeding more manageable. It is also generally advised that women who experience this side effect should take iron supplements to combat anemia. Skipped periodAlternatively, the body may skip periods in the course of adjusting to oral contraceptive use. While this is relatively better than bleeding, skipped periods can leave a woman feeling bloated. Decreasing sodium intake can minimize water retention and the bloated sensation. However, if pill use is inconsistent or there are times of missed pill intakes, medical attention should be sought especially if a woman engaged in unprotected sex for possible pregnancy. NauseaDue to increased hormones in the body, a woman may feel dizzy and nauseous when taking oral contraceptives. The best way to manage this is to chart when the dizzy spells come or what triggers the nausea. For example, if dizzy spells are experienced an hour or two after taking the pill, a woman should consider changing the schedule of her pill intake to a comfortable time like before bedtime. Carrying smelling salts or soothing aromatherapy oils can make passing dizzy spells easier. Weight changeAlthough some women lose weight when taking oral contraceptives, a larger number reports experiencing weight gain when taking birth control pills. While studies show that there is no causal relationship between weight gain and oral contraception, they indicate that a large percentage of the reported increase in weight of pill users are based on how women react to the mood swings and other side effects of pill use. Women can manage their weight when taking oral contraceptives by exercising portion control, staying active, and maintaining a healthy diet. The side effects listed above are commonly experienced by many pill users. They are also minor and are easily manageable. However, if any of the following side effects are experienced, a woman is advised to seek medical attention and talk to her gynecologist or clinician about other birth control options: * severe chest pains or shortness of breath * severe and persistent abdominal pain * frequent migraine attacks or throbbing headaches * moderate to severe leg pains * vision impairment

Heartburn: The Burning Sensation

Heartburn: The Burning Sensation

Heartburn, also called pyrosis, is a condition wherein there is a painful or burning sensation in the esophagus, just below the breastbone because of gastric acid regurgitation. The pain is described to rise to the chest and may even radiate to the neck, throat, or angle of the jaw. The pain worsens when you bend over, lie down, or eat. It may also be more frequent or worse in the evenings. A common misconception is that heartburn affects the heart. The truth of the matter is, this medical condition is not, in any way, related to the heart. One reason why it was called heartburn is because the pain is usually felt on the breastbone where the heart is situated. Another reason is because the hydrochloric acid from the stomach goes back up to the esophagus because of a problem with the cardiac sphincter, a valve in the stomach, which does not have anything to do with the heart. What causes heartburn? To understand what causes heartburn, one must understand how the upper digestive tract works. When you swallow, the cardiac sphincter relaxes to let foods and liquids into the stomach. Then it closes again to prevent food from coming back up. If the cardiac valve weakens of relaxes abnormally, gastric acid may flow back up to the esophagus, causing a burning sensation, or heartburn. Here are factors that may trigger or make heartburn worse: o Food. (fatty foods, spicy foods, chocolate, caffeine, onions, tomato sauce, carbonated drinks, and mint.) o Alcoholic beverages. o Large meals. o Lying down too soon after eating. o Medications. (certain sedatives, antidepressants and calcium channel blockers for high blood pressure) o Cigarette smoking. o Stress and fatigue. o Medical condition. (hiatal hernia) What medical complications may arise from leaving heartburn untreated? Most heartburn attacks are occasional. However, if your heartburn becomes severe or chronic, your body may already be telling you that you have gastroesophageal reflux disorder (GERD). People might mistake it as just a prolonged case of acid reflux. Although heartburn is a symptom for GERD, in this case stomach acid, and occasionally bile flows back into the esophagus, causing inflammation and irritation of the esophagus. Continued irritation may cause narrowing of the esophagus, ulceration of the esophagus, and a slightly increased risk of developing esophageal cancer. How is heartburn treated? Treatment for mild heartburn is more concentrated on providing relief from the symptom. The use of the following medications is advised: o Antacids. These medications neutralize digestive acid and provide quick relief for the burning pain. o H2 receptor blockers. These medications reduce acid production instead of neutralizing the gastric juices. o Proton pump inhibitors. These medications block acid production to allow enough time for esophageal tissue healing. For cases of persistent and prolonged heartburn that may have already developed into GERD, prescription-strength medications are required as medical treatment, and for severe cases of GERD, surgery.Preventive tips A simple lifestyle change may help prevent experiencing the pains of heartburn over and over. Here are some quick tips to reduce the risk of experiencing heartburn. o Watch your weight. Excess pounds mean added pressure to the abdomen, pushing up the stomach and causing acid reflux back up to the esophagus. o Eat smaller meals. Meals in smaller portions reduce pressure on the cardiac sphincter, and it helps prevent the valve from opening and acid from going back into the esophagus. o Eliminate triggers. Take note of the food that trigger your heartburn and try to stay away from them. o Avoid lying down immediately after a meal. Leave a window of two to three hours after eating before going to bed. o Avoid smoking. Ingesting air during smoking aggravates belching and, in turn, acid reflux. Heartburn isnt as simple as it sounds, so dont take it lightly. Your pain tolerance may tell you its nothing but a bad base of belching, but it may lead to further "medical complications" if you leave it unchecked. You are in-charge of your own body, so keep close watch and maintain a healthy lifes.
